Long-Term Care Ombudsman
Our program exists to protect the voices and rights of residents in nursing homes and long-term care facilities. If you live in a nursing home or have a loved one who does, we are here to protect your rights and make sure you receive the care you deserve.
If you or a loved one are facing challenges in a long-term care facility, our team is here to listen, support, and stand by your side.
care you deserve.
Facing challenges with your care?
This service is offered at no cost to residents and families across these Kentucky counties: Breckenridge, Bullitt, Grayson, Hardin, Henry, Jefferson, Larue, Marion, Meade, Nelson, Oldham, Shelby, Spencer, Trimble, and Washington — covering 11,273 beds in 185 facilities.
What we do
We empower families to ask the right questions, evaluate care options, and access reliable comparison data. Start with Medicare’s Nursing Home Compare to explore trusted facility ratings and reviews.
Ombudsmen are guided by the Older Americans Act and are responsible for:
- Identifying, investigating, and resolving complaints from or on behalf of residents.
- Informing residents about their rights and how to exercise them.
- Educating families, facility staff, and the public on long-term care issues.
- Encouraging family and resident councils to strengthen community voices.
- Advocating for policy and practice improvements to enhance quality of care and life.
If you have any questions please email Erika Rhodes-Chism, Director, Long-Term Care Ombudsman program.
Become an advocate
Stand up for dignity.

As a Volunteer Ombudsman, you’ll receive specialized training and support to help ensure nursing home residents are treated with respect, compassion, and fairness. Family Councils
Family members play a vital role in shaping care. A Family Council allows loved ones to meet, share experiences, and advocate for residents collectively. We can help you start or strengthen a Family Council in your facility.
