12 Panel Drug Screening
Performed by ADDT – 820 S 6th Street, Louisville, KY
Our office will provide you with necessary forms
Cost: $30

Medical Records
Medical interpreters must provide proof of same immunizations required for health care workers

  • Tdap (tetanus, diphtheria, pertussis) – Booster every 10 years
  • MMR (measles, mumps, rubella) – 2 part vaccine required
  • Varicella (chicken pox) – 2 part vaccine required
  • Influenza (flu shot) – annual renewal required
  • TB Screening (tuberculosis)- annual renewal required

Continuing Education Credits
Medical interpreters are required to earn a minimum of 4 continuing education credits per year

Background Check
Performed by Catholic Charities includes: criminal history, national sex offender registry, and office of the inspector general

Additional Items
Photo ID & Work Authorization, high school or college diploma, and interpreter orientation/contract agreements
